Hello
I would like to display a submission form in frontend with several fields in a row, with a tabble if possible.
Is there an option to do this ? An extension ? Or is it possible to insert code and where ?

sorry i don't understand .... can you post a screenshot ?
you can use html form field => you can add tab, fielset or other html class in a form for a specific type
FLEXIcontent is Free but involves a very big effort on our part.

For exemple, how can I do a submission form in frontend that contains 14 fields (checkbox), in a table with 2 rows an 7 columns, like attach file :

use that i say and add html in your form
