Problem with Tag URLs with ACEsef

I know this might not be an issue that should be posted here but I thought I'd find out if anyone else has faced this problem and got a solution.

I use ACEsef but the urls for the tags which are generated seem to be behaving crazily. For instance some of the correct tag urls are:

www.feastguru.com/mumbai/tags/malad-west.html
www.feastguru.com/mumbai/tags/mediterranean.html

However, some of the URLs for tags generated include the category or the menu name. For instance:

www.feastguru.com/mumbai/feastguru-servi...lian-restaurant.html
www.feastguru.com/mumbai/reviews/tags/andheri-west.html
www.feastguru.com/mumbai/articles-for-fo...central-suburbs.html

Any idea why this is happening and how this can be stopped?

AceSEF ver 1.3.4
FLEXIcontent extension for AceSEF - 1.3.5
Flexicontent - version 1.5.2 stable r180

In AceSEF Extensions section have you tried setting the the Skip prefix/menu to Yes. If you do it you'll also need to purge the Flexicontent SEFs in AceSEF.

I dont think the problem lies there. My settings are Skip is at "no" and the custom prefix is "mumbai".

The issue is some tags are generated correctly and some are not.

Without seeing the setup in the backend it's kind of hard to diagnose the problem. Almost seems like there's some cross linking items to categories or something on that line.

But the skip No means it will inject an item's alias into the url.
