I didn't move them or copy them, I just changed the primary category in the item settings. Could that be the issue?

Yes, the menu items are adding a "current" category id to the URL e.g cid=45 ,

this is needed because
-- an item may belong to many categories
-- and also category parameters and category status are used when displaying the item,

so if you changed the item's primary categroy the menu items will not be matched

That makes sense. I'll update all the menu items.

Next question (which, I guess, was actually where the issue started) is, what about those pages that have no menu item.

E.g. our news section, where the menu item is something like /intra/news/.

We have four sections of news, where the above mentioned has a category called "News, intra".

For items the order of trying to find a menu item is:
- menu item pointing to the item with correct cid
- menu item pointing to the item's main category (cid and id variables or segments /.../ appear in the url)
- any menu item pointing to one of item's categories is choosen,(cid and id variables or segments /.../ appear in the url)
- the default flexicontent menu item if this is configured in global option


(the above applies, provided the menu item is published and has access level that is allowed to current user)

A hint:

- if your items is in multiple categories to e.g. to catA and catB

when selecting the FLEXIcontent item in Joomla menu item you can
-- force to link via catA if you set category filter to catA
-- force to link via catB if you set category filter to catB
