createDefaultFields fails on blank install J 4.1.2 - FC 4.0.2

Could it be a WAMP issue?
Where did you try this, Karen?
On a Linux or Windows webserver?
My issues were while setting it up on a WAMP installation.

From yesterday i have been trying to fix it.  I have used Chrome, Firefox, Microsoft edge. I have also tried incognito and private mode in all browsers, nothing is working.
I have notice this error, screenshot attached.

 If i provide the login details would you be able to have look please?
