Remove Category from html <title>

Remove Category from html <title> was created by unleash.it
I could swear that I once saw a post addressing this, but now that I needed it I couldn't find it anywhere...

Anyone know how to stop the behavior where the category is sometimes prepended to the html title? I have standard menu items like "About Us" and "Contact" where the title looks like: uncategorized - about-us

Nothing I've done including using a title manager plugin seems to remove the category from these pages.
