Your first code include the second. The first code is the "page title" and the "title".
The second is only the "page title".
Change the first by the second is the same that delete the second ("title").
It's correct?
So I change the codes. Results: title field appears!!
I can make this change in default template.

Pardon, I don't undestand yor question.
"your are in content view ? not category ?"
I enter in the sistem with username and a password of a administrator user in the frontend, there are an exclusive menu item for each admistrator user.
Each one link your option and appears the form to submit articles. In this moment don't can appears Title field
