I have tried with 800px, 500px that doesn't make any difference.

You HTML tags seem not be close properly

Do you have a Joomla template override, for the frontend form?

If so try without the override

Yes the closing tag for <ul> is missing

a </ul> should be placed at line 807 (v2.0 RC8/RC9) just before </fieldset>

but i am not sure if that is effecting you page, i ll check the valid of form HTML again, thanks for feedback
