since you updated web-site from r1932, click CTRL-F5 (most browser) once in item form to make sure you have latest JS/CSS

I found the origine. It's due to my template: ol_logez.
When I choose another one, it's ok.

I still have to find what can cause that on a template.

I asked to olwebdesign, the creator of the template. They found the solution.

I had to select "No" for the parameter "No conflict script".
